Vet Consent Form (not needed for recreational swimming)
To assure that swimming is a safe form of exercise for your dog, if your dog is injured, pre-or-post surgery, or under a veterinarian’s care for any condition, please download and complete the Veterinarian Clearance Form with your registration packet. (We require a Veterinarian approval before swimming any dog that is pre- or post surgery.)
